Before surgery, you will need to contact your insurance company. You will need to find out if pre-authorization, pre-certification or a second opinion is required. It is very important to make this call, as failure to clarify these questions may result in a reduction of benefits or a delay of surgery. (If you have Medicare as your primary insurance, you do not need to call for pre-authorization.)

If you are a member of a Health Maintenance Organization (HMO), you will go through the same registration procedure. However, you will need to call your HMO once your procedure has been scheduled to arrange for pre-admission lab studies that must be completed.

If you do not have insurance, please notify the registration staff when they call you for pre-registration. They can assist you in making payment arrangements.
