We participate in a number of insurance plans. All primary and secondary medical claims will be filed for you, based on the insurance information you have provided. We ask that you assume responsibility for understanding the requirements of your specific insurance plan.
At each appointment our staff will confirm your insurance information. Please bring your current insurance card and co-pay with you to each and every visit. Please refer all insurance coverage questions to your insurance company.
