Making an Appointment
Appointments can be made by calling our clinic Monday thru Friday, 8 am – 5 pm. Please have all necessary information available prior to calling so we can schedule the appropriate time needed for your visit.
Note: If you have a medical emergency that does not require emergency services, go to the clinic and you will be worked into the provider’s schedule immediately.
Please plan to arrive a few minutes early for each appointment. If you have not been seen in our clinic before, please plan to arrive approximately 10 minutes early to fill out your initial information and a health questionnaire. Or, download the forms from our website, complete them at home and bring to your first visit.
Please make sure you have contacted your former provider to transfer your family’s medical records. Bring your child’s immunization record with you so that we can copy the information in to our medical records.
Rescheduling Your Appointment
Please call us if you are unable to keep your appointment. This allows us to facilitate the requests of other patients who are in need of our services. We understand that you may occasionally forget your appointment or that your condition may improve and an appointment is no longer necessary. Please notify us at least one day in advance if you cannot keep your appointment. If you have missed an appointment and still need to be seen, please call your provider’s scheduler.
