We appreciate you scheduling an appointment during regular office hours, Monday – Friday, 9:00 a.m. – 4:30 p.m. We will do everything possible so you can see a physician or nurse practitioner. Please arrive 5 minutes early for each appointment. If you are new, allow 15-minutes prior to your appointment in order to complete new paperwork, which will need to be filled out prior to seeing one of the clinicians. This will provide enough time to fill out the necessary forms and provide us with your health insurance information.
Please bring all medications or a list with you to every visit, whether prescribed by us or by another physician. Make sure you also let us know about any non-prescription medications or remedies you’re using.
