HealthConnect at Home, Home Health and Hospice is a program, affiliated with Saint Elizabeth Regional Medical Center that provides physical care as well as education to patients and their families in the comfort of their home. The HealthConnect team works in cooperation with your physician to provide the highest level of nursing and therapy services. Home care is available to patients of all ages when ordered by a physician.  Nursing staff are on call for patients and families 24 hours a day, 7 days a week.

Referrals
Referrals to HealthConnect at Home may be made by anyone including physicians, social workers, nurses, family members, friends or other health care professionals. But do remember, home care services requires a physician's order.
 
Payment
Medicare, Medicaid, private insurance and self-pay are all payment options for home care services.
