The Saint Elizabeth CARE office helps patients complete as much paperwork as
possible before coming to the medical center. A nurse or other trained healthcare
professionals from the CARE office will contact you before your scheduled surgery
or major procedure to review your health status and medical history, provide information
on how to prepare and answer all your questions. Many of our admission forms are
available online, so you can easily familiarize yourself with them and discuss
them with others if you would like, in advance of your hospital stay. Procedure
information sheets with helpful information on how to prepare for your surgery
